The Department of Health and Social Care (DHSC) has put together a survey to understand the experience of individual employers, personal assistants, and providers who have received Personal Protective Equipment (PPE) via their local authority or local resilience forum.

They will use your answers to improve communications about PPE, raise the quality of PPE delivery, and to discover any problems which need resolving.

DHSC would like to invite you to answer the survey if you have any involvement with PPE as a provider, personal assistant, or individual employer (e.g. you wear PPE, you manage PPE etc.), and you have received PPE from your local authority or local resilience forum.
